29 CFR  1910.178(l)(2)(ii):  The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a. Production Floor: On or about May 18, 2023, and at times prior, the employer exposed employees to struck-by hazards, in that powered industrial truck (PIT) training did not include formal training for employees operating the "Halla" model LF30C (LP) forklift (serial number 5C31097) to stack, store, or relocate 300 gallon chemical totes.

29 CFR  1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:  a.  Production Floor: On or about May 18, 2023, and at times prior, the employer exposed employees to electrical hazards, in that the electrical cord for the "Astro" 9425 dual temperature heat gun was used with a splice which was made for the purpose of changing the plug on the appliance.
