Employees were exposed to unexpected release of hazardous energy.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: On or about May 18, 2023, and or at times prior thereto, in the control room of the dump shed, electrical extension cords ran under a metal door, through a metal wall opening and were not protected from accidental damaged. The employees were exposed to electrical shock hazards.
