29 CFR  1926.25(a):During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  On or about May 17, 2023 the area in front of a ladder used to access the roof was littered with roofing debris with protruding nails, exposing employees to tripping hazards and foot puncture injuries.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about May 17, 2023   employees engaged in residential roofing operations were exposed to struck-by hazards while operating pneumatic nail guns without eye protection.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about May 17, 2023 the employer failed to ensure that employees engaged in residential roofing activities were actively utilizing fall protection.  Employees were engaged in residential roofing activities on a duplex dwelling at a height above 18 feet and were not attached to fall protection.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 17, 2023 the employer failed to provide employees the proper training to identify an mitigate the hazards associated with falls in construction.  The employees were conducting residential roofing activities at a height above 11 feet without utilizing any fall protection.  The employees did not have the knowledge for identifying or mitigating the fall hazards.  Abatement documentation is required for this item per 29 CFR 1903.19(d).
