29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling:  A) The catwalk system in the McChord Boiler Plant had several walkways that were open sided. Employees working on boilers from the walkways on the catwalk system with no fall protection were exposed to fall hazards of 15 to 40 foot heights.

29 CFR  1910.107(g)(2): Cleaning. All spraying areas were not kept as free from the accumulation of deposits of combustible residues as practical, with cleaning conducted daily if necessary:  A)     In the Paint shop, there was a spray booth with accumulations of paint residue/overspray that created a rough surface over the walls of the booth.  B)     In the Structures shop, there was a small open-faced spray booth with accumulations of paint residue/overspray that created a rough surface over the walls and work surface of the booth.

29 CFR  1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work.  Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  A)     In the HVAC shop, there was a Promax bench grinder that had its work rest adjusted to one-quarter inch from the surface of the grinding wheel.

29 CFR  1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel.  The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch:  A)      In the Welding shop, there was a bench grinder where the tongue guard was not properly adjusted.  The guard was seven-eighth inch from the surface of the grinding wheel.
