29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location: Worksite  On or about May 18, 2023, employees were exposed to falls of over 8 feet, as well as eye and head injuries. The employer did not have a competent person to oversee and inspect work. No fall protection was utilized.

Location: Worksite  On or about May 18, 2023, employees were exposed to struck by hazards from flying debris, while using a table saw, and were not protected by safety glasses.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)   Location: Worksite  On or about May 18, 2023, employees were exposed to fall hazards greater than 8 feet from a rooftop to the ground below, where untrained employees were unable to recognize or avoid the hazards, or to wear and use personal fall arrest systems. The employees were not trained on ladders, fall hazards, fall protection, or PPE, and did not receive any other health and safety training from the employer.
