29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about May 19th, 2023 Location: Carol Nichols Park in Elmsford, NY 10523  a) Employee installing side roof molding with battery powered drill while other employees were moving in and out under the drill work area without hard hats.   b) Roof contained unfasten plywoods and power tools that can slide down and strike employees working on the ground level.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about May 19th, 2023 Location: Carol Nichols Park in Elmsford, NY 10523  a) Employees used an extension cord that was damaged at the terminal end by either pull and/or strain to the cable. Strain relief device was missing from damaged end.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  On or about May 19th, 2023 Location: Carol Nichols Park in Elmsford, NY 10523  a) Employee was exposed to fall while engaged on roofing work of about 10 ft without the use of personal fall arrest system or any other type of fall protection. No fall protection was present on site  at the time of the inspection.
