29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. (a) Bridgeport Fire Department, Exterior: On or about May 17, 2023, an employee was working from a height of approximately 18.6' while on a scaffold laying block. There were no means of fall protection utilized to protect the employee from the open right side or open portion of the front side.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: (a) Bridgeport Fire Department, Exterior: On or about May 17, 2023, an employee was working from a scaffold to lay block. While on the third buck of a scaffold, the employee stood on planks that were placed across two cinderblocks in order to increase the work level height to reach the last course of block.
