29 CFR 1903.19(c)(1): Employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:   Location:  Grasso Hall Westside Campus WCSU, 43 Lake Avenue Ext., Danbury, Connecticut  Abatement certification and verification for Citation 001, Item 2 with a final order date of 8/18/2023 was not received within 10 calendar days (by 7/17/2023) following the abatement due date ( 7/7/2023).

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Roof top Grasso Hall: On or about February 14, 2023 the employees were exposed fall hazards of up to 48 feet above the ground while installing new roof flashing on the flat roof.  The Three Brothers Service LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1593024, citation number 1, item number 3a and was affirmed as a final order on August 8, 2022 with respect to a workplace located at 6 Arch Street, Norwalk, CT 06850.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Roof top Grasso Hall: On or about February 14, 2023 the employer did not ensure that the employee who was required to work from the roof of the building at a height of approximately 48 feet were trained in fall hazards and the procedures to be followed to minimize such hazards.  The Three Brothers Service LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1593024, citation number 1, item number 3-b and was affirmed as a final order on August 8, 2022 with respect to a workplace located at 6 Arch Street, Norwalk, CT 06850.
