29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a) Front entrance to structure: On or about 05/18/2023, and at times prior, the employer exposed employees to ejection/fall hazards while working from JLG 460SJ Aerial Lift, in that the employee was not provided with a body belt and lanyard to attach to the anchor points provided in the basket while engaged in metal grinding and welding framing activities.

29 CFR 1926.303(b)(1): Grinding machines shall be equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section. a) Front side of structure: On or about 05/18/2023, the employer exposed employees to struck-by/amputation/laceration hazards, in that the employer did not provide the employee with machine guarding for Metabo angle grinder with exposed 6-inch grinding disk while grinding and welding steel framing. b) Front side of structure: On or about 05/18/2023, the employer exposed employees to struck-by/amputation/laceration hazards, in that the employer did not provide the employee with machine guarding for DeWalt angle grinder with exposed 6-inch wired grinding wheel while grinding and welding steel framing.
