29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Worksite roof - On or about May 18, 2023 and at times prior thereto, employees were performing roofing work approximately 28 feet above the ground while not using adequate personal fall protection; employees were using a rope grab system with the rope tied around their waists, employees were not tied off to an anchor point, and employees had no fall protection in place.

Employees were not provided with fall protection training .

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a) Porch roof - On or about May 18, 2023 and at times prior there, employees were accessing the main roof from a ladder that was positioned on the peak of the porch roof, with a side rail on each side of the peak approximately 14 feet above the ground. The ladder was not secure.
