On or about 5/23/2023, the employer did not ensure that employees working on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling. Employees were working the edge of Makino T2 CNC machine without any protection from falling.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  JOB SITE: 4800 Baumgartner Rd, St Louis, MO. On or about 5/23/2023, the employer did not establish a program consisting of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on the CNC machines, where the unexpected energizing, startup, or release of store energy could occur and cause injury, the machines were isolated from all energy courses and rendered inoperative. Employees were exposed to crush hazards due to preforming maintenance on Haas CNC machines without conducting any energy control procedures.

On or about 5/23/2023, the employer did not ensure natural or synthetic fiber rope sling(s) with discoloration or rotting were  immediately removed from service. Employees were exposed to struck-by hazards due to using polyester slings that were discolored and frayed to raise and lower parts into and out of a CNC machine.
