29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On May 23, 2023 and at times thereto, a Rightline Rotator was added to the Yale forklift #10052429. The forklift is used to dump boxes of beans weighing up to approximately 3800 lbs. (b) On May 23, 2023 and at times thereto, a fork on the Hyster #55 truck, was modified with a hole cut into the tip of one of the forks. Note: Abatement certification and documentation is required for this item.
