29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Location: 3rd floor On or about May 24th, 2023 employees were exposed to slips, trips, falls, and puncture injuries while walking around where lumber cut offs and 2x4s with protruding nails were not kept clear of work areas and walking pathways.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: On the Top Floor Deck On or about May 24th, 2023, an employee was exposed to falls from heights up to 26 feet to the ground while staging plywood on the joists where fall protection was not utilized.
