On or about May 24, 2023 at the address of  1255 South Byrne Rd. Toledo OH 43614: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Toledo OH 43614: The employer did not ensure that employees were protected from head injury hazards by protective hardhats while working on the ground below roof line as employees above actively remove/replace roofing materials. Employee(s) operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and roofing materials.

Toledo OH 43614: The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a two-story multi-family residential structure with a 5 -12 roof pitch. Employees operating in this conditions were exposed to fall injuries from heights of 18 feet to the ground below.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  a) Platinum Roofing and Construction LLC. On or about May 24, 2023 at the address of  1255 South Byrne Rd. Toledo OH 43614: The employer did not ensure employees using aluminum ladder had their side rails extended at least three feet above the landing surface. Five employees performing roofing work activities were exposed to fall hazards while using ladder to access/exit roof on a two-story residential structure with a height of 18ft from ground below.

On or about May 24, 2023 at the address of  1255 South Byrne Rd. Toledo OH 43614. The employer did not provide a training program for each employee potentially exposed to fall hazards while using an aluminum extension ladder to access a two-story residential roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.
