On 5/24/2023, Campbell Sleep, LLC located at 100 Minnesota Ave, Cape Girardeau MO, had not ensured any powered industrial truck which was found to be in need of repair, defective, or in any way unsafe, was removed from service. The following forklift was in use and not removed from service: a) Hyster 30, SN C101H03767U, Model S30XM. The forklift was defective at the time of the inspection; it had an unserviceable LPG locator/indexing pin.
