29 CFR 1910.178(p)(1):  Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition.  On 5/24/2023, Campbell Sleep, LLC located at 100 Minnesota Ave, Cape Girardeau MO, had not ensured any powered industrial truck which was found to be in need of repair, defective, or in any way unsafe, was removed from service. The following forklift was in use and not removed from service:  a) Hyster 30, SN C101H03767U, Model S30XM.  The forklift was defective at the time of the inspection; it had an unserviceable LPG locator/indexing pin.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.  On 5/24/2023, Campbell Sleep, LLC located at 100 Minnesota Ave, Cape Girardeau MO, employees engaged in manufacturing activities were exposed to burns, electrical shock, and electrocution in the southwest corner of the production area that had a missing outlet cover exposing electrical wires.

The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  On 5/24/2023, Campbell Sleep, LLC located at 100 Minnesota Ave, Cape Girardeau MO, did not certify that each forklift operator has been trained and evaluated.  The employer did not have a certification, which included the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.
