OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, explosion, and/or deflagration hazards from a dust collection system that was not equipped with devices and systems to prevent the propagation of deflagration and associated flame fronts through the collection system ductwork and back into the building and further creating larger dust explosions/deflagrations from the additional fuel load of dust in the building:  A.) Molded Fiber Glass dba MFG Union City, Manual Trim Area: On or about February 22, 2023, and at times prior thereto, employees were potentially exposed to fire, burn, and deflagration hazards from a cyclone, and the associated ductwork, that were not equipped with devices and systems to prevent the propagation of deflagration and associated flame fronts from the cyclone through the associated ductwork, back into the building, and to the dust collection bag. The cyclone processed combustible fiberglass and plastic dust.  B.) Molded Fiber Glass dba MFG Union City, Manual Trim Area: On or about February 22, 2023, and at times prior thereto, employees were potentially exposed to fire and burn hazards from a cyclone, and the associated ductwork, that were not equipped with devices and systems to detect and suppress fires, sparks and embers within the cyclone and/or ductwork.  The cyclone processed combustible fiberglass and plastic dust.  C.) Molded Fiber Glass dba MFG Union City, Manual Trim Area: On or about February 22, 2023, and at times prior thereto employees were potentially exposed to fire and burn hazards due to the presence of the dust collection system's collection bag in the Manual Trim Room, at the end of an exhaust duct.  The collection bag contained dust and severed as a fuel load for a potential dust explosion.
