29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a. On or about May 23, 2023, at the above addressed jobsite, employee(s) performing residential chimney repair services from a tubular welded frame supported scaffold, approximately twelve (12) feet eight (8) inches above ground, did not have the working level fully planked, thereby exposing the employee(s) to fall hazards. b. On or about May 23, 2023, at the above addressed jobsite, employee(s) performing residential chimney repair services from a tubular welded frame support scaffold, standing on a bracketed extension platform, approximately sixteen (16) feet above the ground, did not have the working level approximately three (3) feet beneath the bracketed extension platform fully planked, thereby exposing the employee(s) to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about May 23, 2023, at the above addressed jobsite, employee(s) performing chimney repair services from a tubular welded framed support scaffold between twelve (12) feet eight (8) inches to sixteen (16) feet above ground, were exposed to injuries from falls due to lack of a complete guardrail system. The scaffolding was erected without endrails, midrails, and guardrails where employees were exposed to falls.
