Employees engaged in forklift operations at 2516 Deer Park Blvd, Omaha, NE 68105, were exposed to exposed to crushing and struck-by hazards in that employees were using a  forklift  attachment that was not pre-approved by the manufacturer.  Employees were using the forklift attachment to move and stage trailers.
