29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees engaged in maintenance activities at 2516 Deer Park Blvd, Omaha, NE 68105, were exposed to struck by hazards in that employees were using a bench grinder and the work rest was not adjusted to 1/8 inch.

29 CFR 1910.178(a)(5):If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Employees engaged in forklift operations at 2516 Deer Park Blvd, Omaha, NE 68105, were exposed to exposed to crushing and struck-by hazards in that employees were using a forklift attachment that was not pre-approved by the manufacturer. Employees were using the forklift attachment to move and stage trailers.
