29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about May 25, 2023, at a residential building located at 454 Wood Street in Oconomowoc, Wisconsin, employees were exposed to fall hazards in that: (a) An employee on the Southeast upper roof, conducting tear off work, was not protected from a fall hazard of approximately 19.5 feet; (b) An employee the lower roof (above the entry), conducting tear off work, was not protected from a fall hazard of approximately 9.5 feet, and; (c) An employee the West garage roof, conducting tear off work, was not protected from a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about May 25, 2023, at a residential building located at 454 Wood Street in Oconomowoc, Wisconsin, employees were exposed to fall hazards in that: (a) The portable extension ladder being used for access to the roof on the two-story residential home did not extend at least 3 feet above the landing surface, exposing the employee to a fall hazard of approximately 19.5 feet; (b) The portable extension ladder being used for access to the roof on the residential garage did not extend at least 3 feet above the landing surface, exposing the employee to a fall hazard of approximately 10 feet.
