29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.) Mike Tusin 1042 Ridge Ave, Sharpsville, Pa: On or about 05-24-2023 and at times prior thereto, employees were conducting roofing operations on a residential roof with an 8:12 pitch without any fall protection systems. Employees were exposed to fall hazards of 9-feet to 20-feet above ground level.

Mike Tusin 1042 Ridge Ave, Sharpsville, Pa: On or about 05-24-2023 and at times prior thereto, Employees were performing roofing activities that include, but not limited to, shingle removal and installation.  Employees were exposed to fall hazards of 9-feet to 20-feet above ground level.  Employees did not have fall protection training.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.) Mike Tusin 1042 Ridge Ave, Sharpsville, Pa: On or about 05-24-2023 and at times prior thereto, the Gorilla metal ladder that was utilized to access the 2nd story roof from the porch roof was only 1-foot above the roof line at approximately 20-feet above ground level.  b.) Mike Tusin 1042 Ridge Ave, Sharpsville, Pa: On or about 05-24-2023 and at times prior thereto, the red fiberglass ladder that was utilized to access the 1st story roof at the rear of the house was only 2-feet above the roof line at approximately 9-feet above ground level.
