29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) On or about May 25, 2023 at 92-54 Queens Blvd, Rego Park, NY 11374 A floor hole cover measuring approximately 3ft long and 8 inches wide that was made of lumber in the ground floor, was not secured, and employees were exposed to a trip and fall hazard.
