29 CFR  1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    163 Maple Grove Drive, Pittsfield, MA: On or about May 25, 2023 and at times prior, Employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges up to 10 feet above the next lower and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  J and Z Roofing and Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) , which was contained in OSHA inspection number 1615745, citation number 2, item number 1 and was affirmed as a final order on 11/30/2022, with respect to a workplace located at 181 Second Street, Pittsfield, MA.  J and Z Roofing and Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) , which was contained in OSHA inspection number 1609076, citation number 2, item number 1 and was affirmed as a final order on 10/19/2022, with respect to a workplace located at 280 Great Road, Shirley, MA.  J and Z Roofing and Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) , which was contained in OSHA inspection number 1548293, citation number 1, item number 1 and was affirmed as a final order on 01/12/2022, with respect to a workplace located at 16 Kathy Way, Pittsfield, MA.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   163 Maple Grove Drive, Pittsfield, MA: On or about May 25, 2023 and at times prior employee(s) were exposed to fall hazards as part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures. 29 CFR  1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.
