At 1218 SW 5th Ave., Gainesville, FL: On May 25, 2023, at 9:30 AM, the employer exposed employees to trenching hazards, in that the employees were in a trench without having a ladder or means of egress to use to get out of the trench.

At 1218 SW 5th Ave., Gainesville, FL: On May 25, 2023, at 9:30 AM, the employer exposed employees to a cave-in hazard in the 6 1/2 foot deep excavation in that no cave-in protective system was used.
