The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about May 4, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, an employee was exposed to machine hazards associated with moving parts during sanitation of a Surge Tank pump on Line #6. The employer did not ensure that the employee implemented energy control application steps as the machine was not shut down or turned off to perform the servicing work per [1910.147(d)(2) requirements]. b) On or about May 25, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, an employee was exposed to machine hazards associated with moving parts during sanitation of a Surge Tank pump on Line #6. The employer did not ensure that the employee implemented energy control application steps as the machine was not shut down or turned off to perform the servicing work per [1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about June 12, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not ensure that the in-running nip point of the transfer conveyor to the Tumble Drum on the shred line had been guarded to prevent exposure to the caught-in hazard. b) On or about June 12, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not adequately guard the forming head of the A.E. Randles Co., Inc. Tray Former Model 16-26, Serial #VF-3099 to prevent exposure to the struck-by hazard while employees added glue to the glue pot of the machine. c) On or about June 12, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not adequately guard the forming head of the A.E. Randles Co., Inc. Tray Former Model 16-26, Serial #VF-3120 to prevent exposure to the struck-by hazard while employees added glue to the glue pot of the machine. d) On or about May 25, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not guard the access door where employees add material to the cooker on Lines 1-6, 8-9 & 11, exposing them to potential entanglement hazards from the rotating auger inside the cook tank. e) On or about May 25, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not guard the Cooker Tank outfeed hatch on Lines 1-6, which exposed employees to potential entanglement hazards from the rotating auger inside the cook tank while taking samples.

The controls for the barrel cutter allowed employees to operate the machine while still being able to reach the point of operation. c) On or about June 12, 2023, with regard to an establishment located at 225567 Silver Maple Lane Colby, WI 54421, the employer did not ensure that employees were protected from potential burn hazards while working with the Bag Starter in the Line 5/6 Packaging Department.
