(a) Location - 401 Lowell Avenue, Haverhill, MA On or about 05/17/2023, The employer did not have a survey from a competent person of the stability of the commercial structure to be demolished before beginning demolition operations, exposing the employees to the crushing and collapse hazards from the unstable structure.
