1926.20(b)(2) The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: a competent person did not determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. (a) Location - 401 Lowell Avenue, Haverhill, MA On or about 05/17/2023, The employer did not have a survey from a competent person of the stability of the commercial structure to be demolished before beginning demolition operations, exposing the employees to the crushing and collapse hazards from the unstable structure.
