29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  a.  On or about May 24, 2023, at the above addressed jobsite, an employee performing siding installation operations accessed a roof by climbing the guardrails of an aerial lift when transitioning from the basket to the roof's surface, thereby exposing the employee to fall hazards.

On or about May 24, 2023, at the above addressed jobsite, an employee working from a SkyJack SJ 66T aerial lift was not wearing a personal fall arrest system, thereby exposing the employee to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about May 24, 2023, at the above addressed jobsite, an employee performing siding installation operations, ten (10) feet three (3) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  Straightline Exteriors LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1411859, citation number 1, item number 1, and was affirmed as a final order on August 8, 2019, with respect to a workplace located at 518 Cahokia St. in Joliet, Illinois.
