Condition was noted on or about May 26, 2023.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Jobsite located at 1522 N. 4th Street, Philadelphia, PA. - The employer did not ensure that employees accessing the bottom of a wall opening on the 2nd level of a 3 story residential building, approximately 15 feet above the ground level, were provided with safe access by extending the extension ladder at least 3 feet above the upper landing surface of the wall opening. Condition was noted on or about May 26, 2023.
