29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On or about May 26, 2023, and at times prior thereto, an electrical lift at the outer dock had damage to the outer jacket of the cord and plug connected equipment and was not removed from service, exposing employees to electrical hazards.
