29 CFR 1926.451(d)(3)(i):Before the scaffold was used, direct connections were not evaluated by a competent person to confirm, based on the evaluation, that the supporting surfaces are capable of supporting the loads to be imposed: a. On May 25, 2023, at the above addressed jobsite, an employee performing exterior restoration operations from a suspended scaffold, was not inspected by a competent person, thereby exposing the employee to hazards associated with falls of approximately eighteen (18) feet eleven (11) inches.

On or about May 25, 2023, at the above addressed jobsite, an employee performing exterior restoration operations from a two-point adjustable suspension scaffold, approximately eighteen (18) feet eleven (11) inches above the ground, did not utilize a personal fall arrest system, thereby exposing the employee to fall hazards.
