29 CFR 1910.180(d)(5)(ii): A crane which has been idle for a period of six months shall be given a complete inspection conforming with requirements of paragraphs (d) (3) and (4) of this section and paragraph (g)(2)(ii) of this section before placing in service. a. On or about May 25, 2023, the employer did not ensure that the Grove TM225, Serial 661346, 25-Ton truck mounted crane that was idle for the past six months was inspected in accordance with the (d)(3) Frequent Inspection, (d)(4) Periodic Inspection and (g)(2)(ii) Wire Rope Inspection requirements in order to identify deficiencies which could affect the safe operation of the crane prior to hoisting concrete wall barrier blocks to calibrate the AC tank scale , thereby exposing employees to crushed-by hazards.

29 CFR 1910.180(h)(3)(ix): Outriggers shall be used when the load to be handled at that particular radius exceeds the rated load without outriggers as given by the manufacturer for that crane. Where floats are used they shall be securely attached to the outriggers. Wood blocks used to support outriggers shall: a. On or about May 25, 2023, the employer did not ensure that outriggers for the Grove TM225, Serial 661346, 25-Ton truck mounted crane were extended and positioned prior to operating the crane to hoist an approximately 3,400 pound concrete wall barrier block to calibrate the AC tank scale. The crane was overturned as a result of being operated outside the rated load limits established by the equipment manufacturer, thereby exposing employees to crushed-by hazards.
