29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:   Employees engaged in ammonia refrigeration activities at 2223 Industrial Road, Crete, Nebraska on or about May 16, 2023, were exposed to chemical hazards in that the employer did not document that it complied with recognized and generally accepted good engineering practice (RAGAGEP), including ANSI/IIAR Standard 2 - 2014 (Standard for Safe Design of Closed-Circuit Ammonia Refrigeration Systems), Section 5.14.15; IIAR Bulletin 114 (Guidelines for Identification of Ammonia Refrigeration Piping and System Components), Sections 5.0 and 6.0; ANSI/ASH RAE Standard 15 -2019 (Safety Standard for Refrigeration Systems), Section 9.12.6; and IIAR Bulletin 109, Section 4.10.3 (Minimum Safety Criteria for a Safe Ammonia Refrigeration System), Sections 4.7.6 and 4.10.3, when it did not ensure that ammonia system valves located in the engine room and on the roof were clearly labeled.
