29 CFR 1910.133(a)(1):Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: Kitchen: On or about May 25, 2023, eye protection was not provided where chemical capable of causing severe irritation and burns to the eyes, were used. Chemicals include, but were not limited, to Xpress Degreaser and Oven cleaner tablets both containing corrosive sodium hydroxide.

29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Kitchen: On or about may 25, 2023, there was no eyewash providing at least 15 minutes of flow where corrosive chemicals, including XPress Degreaser. oven cleaning tablets, QA Ultra Sanitizer, and Super 8 were used and were capable of cause burns to the eyes.
