OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  On or about May 30th, 2023 employees in the maintenance shop were engaged in tire repair activities and were exposed to explosion hazards due to the separation of the tire from the rim and the employees were not trained on the hazards associated with servicing tires. The result of these failures was an employee struck and killed by an exploding rim and tire during inflation outside of a tire cage.  AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE: i.	Developing, implement, and communicate a training curriculum addressing safely servicing tires with tire cages, use of clip-on chucks and in-line pressure gauges, as well as servicing split rims.  ii.	Develop and implement a safe operating procedure for servicing single and split piece rims.  iii.	Incorporate the use of a restraining device such as a cage or rack for tire inflation to prevent explosive separation or failure of tires/rims.  iv.	Incorporate the use of a clip-on chuck, an in-line pressure gauge, and current manuals/charts for instructions on how to service tires, rims, and innertubes in a safe manner.
