29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.) At the jobsite located at 1554 Florence Blvd. Florence, AL 35630 on or about 05/31/2023, the employer exposed employees to fall hazards in that fall protection was not utilized while wooden trusses were being set on a commercial structure while working at approximately 16 feet in height.

29 CFR  1926.453(b)(2)(iv):Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  a.On or about  May 31, 2023, at 1550 Florence Blvd., Florence, AL 35630, the employer exposed employees to fall hazards in that, fall protection was not utilized by employees, who were working from a JLG Man lift, Model Number 460SJ, where the employees were working at an elevated height where the fall distance was  approximately 8 feet. Employee was observed climbing over handrails of the man lift  to access the roof of the commercial structure.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  a. On or about  May 31, 2023, at 1550 Florence Blvd., Florence, AL 35630, the employer exposed employees to fall hazards in that, fall protection was not utilized by employees, who were performing framing activities  on the side of the commercial structure at approximately 14 feet. The aerial lift fall distance to the ground was approximately 8 feet.
