A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. (a) On May 31, 2023, near 186 Eighth Street SW in Dickinson, ND an employee was exposed to a cave-in hazard while installing a water pipe inside a trench that was more than 4 feet deep without a ladder or other means of egress.

(a) On May 31, 2023, near 186 Eighth Street SW in Dickinson, ND an employee was exposed to a cave-in hazard while installing a water pipe inside a trench without any inspections of the trench done before the employee entered.
