29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  95 Ellington St., On or about May 31, 2023, employees were exposed to fall hazards are part of their assigned duties and the employer had not trained them prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.

The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  95 Ellington St. Longmeadow, MA : On or about May 31, 2023;  employees actively engaged in residential roofing activities were utilizing personal fall arrest systems and the attachment point had not been located  in the center of the wearer's back near shoulder level.  Employees were observed  connected to the positioning rings on the front of the harness.
