29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds).  a) On or about May 25, 2023, at the above addressed jobsite, an employee conducting electrical work on a junction box from a Hyster 120 Fortis Clamp Forklift Truck Model S120FTPRS Serial No. J004V01748N,  approximately nineteen (19) feet above ground was not protected from falling, thereby exposing the employee to fall hazards. The scaffold (i.e., solo cup roll clamped by the clamp of the truck of the forklift) did not meet the requirements of 29 CFR part 1926, subpart L.

29 CFR  1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  a) On or about May 25, 2023,  at the addressed jobsite, the employer did not ensure employee(s) who were required to perform maintenance task that included working on electrical equipment and components were provided with training on electrical safety-related work practices, such as but not limited to electrical shock hazards, arc flash/blast hazards, and protective equipment for the face, hands, and body to protect against electrical hazards while working on or near energized conductors.

29 CFR  1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:  a) On or about May 25, 2023, at the addressed jobsite, the employer did not ensure employees utilized safe electrical related work practices while performing work on or near energized circuits.  An employee suffered electrical shock and electrical burns while working on live electrical conductors.

29 CFR  1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a) On or about May 25, 2023, at the addressed jobsite, the employer did not ensure employees had appropriate protective equipment when working on an energized electric parts and or components .  An employee, maintenance mechanic, was not provided with adequate hand and face protection, thereby exposing the employee to hazards associated with electrical shock and burns.
