On or about: 1/17/2023 Location: 500 Commerce St., Hawthorne, NY 10532 a) Two employees were observed working on the roof of a new multi-residential building within approximately 2 or 3 feet of overhead primary electric distribution lines which measure at 4 kilovolts (KV).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about: 1/18/2023 Location: 500 Commerce St., Hawthorne, NY 105 a) Three (3) workers performing edge flashing and shingle inspection/repair on roof structure were exposed to a serious fall hazard greater than approximately 15-feet due to not being protected by either guardrails, safety nets, or a personal fall arrest system.
