29 CFR  1910.134(d)(3)(ii): The employer did not provide a respirator that was appropriate for the chemical state and physical form of the contaminant:   a) On and before 6/1/2022, in the field, for the employees wearing a 3M half-face respirator with OV cartridges while spraying Shockwave RTU - 8316 containing chemicals such as, but not limited to benzl-C12-18-alkyldimethylammonium chloride, and Sentinel 24-7 Zero Clear Mold & Mildew Resistant Coating containing chemicals such as, but not limited to, zinc oxide.  The employees were not provided with a combo organic vapor/P100 cartridge prior to being required to wear the respirator.

29 CFR 1910.134(f)(2):  The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.   a) On and before 6/1/2022, in the field, for the employees wearing a 3M half-face respirator with OV cartridges while spraying Shockwave RTU - 8316 containing chemicals such as, but not limited to benzl-C12-18-alkyldimethylammonium chloride, and Sentinel 24-7 Zero Clear Mold & Mildew Resistant Coating containing chemicals such as, but not limited to, zinc oxide.  The employees were not provided a medical evaluation prior to being required to wear the respirator.  The employees were not fit tested, either qualitatively or quantitatively, prior to being required to use this respirator by the employer.
