29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about May 31, 2023, at a job-site located at 1135 Wales Rd NE, Massillon Ohio 44646; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with a near vertical side wall and a depth of approximately 6.4 feet. The trench was determined to have type "B" soil.
