29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)   This violation occurred on or about June 1, 2023, where employees doing roof work on a pre-existing two (2) stories single-family house, while walking and working at ground level where an overhead hazard exists by not wearing hard hats and were exposed to head injuries.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)	This violation occurred on or about June 1, 2023, where employees doing roof work on a pre-existing two (2) stories single-family house, while using a DeWalt model no. DCF809 nail gun and were not wearing safety glasses.  Employees were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a)   This violation occurred on or about June 1, 2023, where employees doing roof work on a pre-existing two (2) stories single-family house and were exposed to fall hazards to the lower level.
