29 CFR 1926.453(b)(2)(iv):Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position: On or about May 31, 2023, and at times prior thereto, employees were standing on the top rail of a boom lift while working at elevated heights above approximately 20 feet, exposing employees to fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about May 31, 2023, and at times prior thereto, employees were installing electrical cables while standing on cable trays suspended from ceiling at heights above approximately 20 feet without fall protection, exposing employees to fall hazard.
