29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  Jobsite: 7 Brookline Blvd, Havertown, PA 19083 - On or about June 2, 2023, the employer did not ensure employees conducting masonry operations from a supported tubular welded frame scaffold, approximately 14' 4" above the ground level, were protected from falls by the use of a fall protection system.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Jobsite: :7 Brookline Blvd, Havertown, PA 19083 - On or about June 2, 2023, the employer did not provide a safe means of access to each level of a supported tubular welded frame scaffold by the use of portable ladders, hook-on ladders or other acceptable means. Employees climbed the scaffold end frames to access the 2nd working platform, approximately 14' 4" above ground level.
