29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about May 31, 2023, a roofer was installing roof decking on a residential apartment building under construction without fall protection, exposing the roofer to a fall hazard up to approximately 30 feet high.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  On or about May 31, 2023, four roofers were using the top step of an eight-foot A frame ladder to access the upper levels of the apartment complex under construction, exposing the roofers to a fall hazard.
