29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a) Jobsite, Terraces at Maplewood Multi-family Development, Harrisburg, PA - On or about June 2, 2023, an employee was exposed to a 20-foot fall hazard while installing exterior siding on a new construction residential dwelling working from pump jack scaffolding that did not have side guardrails installed. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite, Terraces at Maplewood Multi-family Development, Harrisburg, PA - On or about June 2, 2023, employees were installing exterior siding on a new construction residential dwelling working from pump jack scaffolding without receiving training from a qualified person on how to recognize hazards with the type of scaffolding being used. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
