Loop 1604 San Antonio Texas, on or about June 1, 2023, an employee was exposed to an unexpected release of stored energy while repairing a semi dump trailer that was struck in a raised position without chocking the wheels and setting all the brakes.

Loop 1604 San Antonio Texas, on or about June 1, 2023, and at times prior thereto, employees were exposed to struck-by hazards while repairing a semi dump trailer without having energy isolating devices available prior to beginning work.
