29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a) 610 Jerry St., Castle Rock, CO. 80104: On or before 06/02/23, First Management, Inc. dba JR Mechanical as creating and exposing employer, exposed its employees and other subcontractor employees at the site to tripping hazards in that, employees installing 4" PVC piping pulled the hole covers on several holes located on floors above where they were installing the piping. The holes were left exposed to other workers at the site.
