29 CFR  1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, was not determined prior to opening an excavation:   a) Job site, behind 118 South Center Street, Somerset, PA: On or about June 5, 2023, during excavation activities, the employer did not ensure that the estimated location of all gas lines that could reasonably be expected to be encountered were determined prior to the start of the work, resulting in a service line being hit by an excavator.
