OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to thermal burns: (a) On June 1, 2023, at the Bean Frack Pad (OTT USA 44-7TFH) located at or near Killdeer, ND, while engaged in servicing/loading activities of flow back water into a water truck, an employee was exposed to thermal burns when stored energy was released into a filter pod resulting burn injuries. Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to: 1. Follow the American Petroleum Institute Recommended Practice 54, Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations section 6.5.3 - which provides examples of energy isolation devices to include but not limited to the use of double block and bleed valve systems. 2. Follow the American Petroleum Institute Recommended Practice 54, Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations section 6.5.4 - which states prior to work on equipment where energy isolation has been applied, the equipment shall be tested to verify (try out) that energy isolation is successful and stored energy has been released.
