29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (Construction Reference 1926.103) a) Hyatt Place, 9030 Westview Rd, Lone Tree, CO 80124: On or about May 24, 2023, and at times prior, employees were exposed to inhalation and breathing hazards in that employees did not receive Appendix D of 29 CFR 1910.134. The employer failed to ensure employees were informed on proper respirator usage.
